When you come for your first consultation, is important that you bring as much relevant information as possible.
Please bring along an up-to-date referral from your doctor. You will also need your Medicare card or DVA card if you have one.
Please bring all relevant x-rays and scans that you may have already had completed.
If your injury is work-related, we will need your WorkCover claim number and employer details.
